Supporting documents Required are:-
a. Application affixed with recent passport size photograph duly signed.
b. Consent letter to work under above mentioned terms and conditions.
c. Registration under Medical Council of India,
d. Relevant Certificates and Qualification certificates
e. Certificates for Experience.
